In order to qualify for this program you must be at least 40 years of age with a low to moderate income, and you may not have Medicare. All women 40 and older who meet the income guidelines listed at the bottom of this page will receive the voucher for the free mammogram. For an appointment, simply call 963-7300 or 468-2797 and receive a coupon for a free lunch just for taking care of your health.

Women 50 years of age and older who meet the income guidelines listed below will qualify to receive all of our exams, including the mammogram voucher, for free. No documentation of income is necessary. Women between 40-49 years of age who meet the income guidelines below will also qualify for the free mammogram voucher, but must pay a small fee for clinical exams based upon household income.
