Request forms for certified birth and death records can be obtained by downloading a printable request from the list below. If you don’t have a driver license or state issued ID, please see the list of identification requirements (24 KB).

Fees are listed on request forms.

Request Should Include:

• A copy of both sides of your state issued ID
• Type of record being requested
• The full name on the record
• Date and city the event occurred
• Parents names including mother’s FULL maiden name
• Your relationship to the person on the record
• Your signature
• Reason for making the request
• Your daytime telephone number is helpful
• Your return address

Please make check or money order payable to "Vital Records." We do not accept credit card orders through the mail. Any request that is not complete or is missing a valid copy of the applicant’s ID or does not have the correct fee will be returned.
