Birth certificates available at SLVHD offices:
1. Salt Lake City limits for the years 1890 through 1950.
2. All births that occurred in Utah from 1951 to the present time.
Records are available to the person listed on the record, immediate family which includes: parents listed on the record, spouse, siblings, children and also the person’s grandparents and grandchildren. Others who want a record for legal purposes will need to show their ID and proof of their legal need for the certificate.
3. If the birth occurred before 1951 in another city in Utah, contact the Utah Department of Health at (801) 538-6105.
