How do I apply for a copy of a vital record in person?

1. Complete the appropriate form: Birth, Death, Marriage Abstract, Divorce Abstract
2. Bring the completed form, along with acceptable identification, to one of our office locations
3. Pay the applicable fee by cash, personal check, MasterCard, or Visa
4. Your request will be processed while you wait

How do I apply for a copy of a vital record through the mail?

1. Complete the  appropriate form: Birth, Death, Marriage Abstract, Divorce Abstract
2. Copy both sides of your acceptable identification
3. Make a check or money order payable to SLCoHD Vital Records
4. Send all three items to:
   SLCoHD Vital Records
   610 South 200 East
   Salt Lake City, UT 84111
5. Your mail request will be completed and returned promptly

How can I expedite my request?

Salt Lake County Health Department does not accept online orders; however, for your convenience, you can process online requests through VitalChek, an independent provider. 

An additional fee is charged by VitalChek for using their service, and they accept all major credit cards.

VitalChek has comprehensive information on state vital records offices throughout the country.

Where can I find statistics on births occurring in Utah?

Statistics are available online from the Utah Department of Health Center for Health Data.
