Children’s Health Insurance Program (CHIP)

The Children's Health Insurance Program, or CHIP is a state health insurance plan for children. Depending on income and family size, working Utah families who do not have other health insurance may qualify for CHIP.
CHIP also includes limited dental coverage for children.

Medicaid is a federal and state funded program run through the Utah Department of Health and Department of Workforce Services. Utah Medicaid helps provide payment for medical/dental bills for those who qualify. In order to receive Medicaid assistance individuals must meet certain income guidelines and cannot afford the cost of health care. Contact the Utah Medicaid office for additional qualification details.
Primary Care Network (PCN)

The Primary Care Network (PCN) is a health plan offered by the Utah Department of Health. It covers services administered by a primary care provider. If you are uninsured and age 19-64 you may qualify for PCN.
PCN provides limited dental coverage including: exams, x-rays, cleanings, and fillings.

UPP (Utah's Premium Partnership for Health Insurance) helps make health/dental insurance more affordable for families and individuals. UPP (pronounced 'up') helps you pay your monthly health insurance premiums through your employer's health insurance plan or COBRA coverage.

The Health Access Project (HAP) provides an essential community service by helping low-income, uninsured residents of Salt Lake County gain access to quality primary and specialty medical care. To be eligible for HAP services, prospective patients must meet the following criteria:

Maliheh Free Dental Clinic

The Maliheh Free Clinic is intended to serve both children and adults in families who do not have dental/health insurance and are not eligible for Medicare or Medicaid. In addition, the household income must be at or below 150% of the Federal Poverty Level.
The Maliheh Clinic only offers dental extractions. Appointments are scheduled for Wednesday mornings and fill quickly. Call to book an appointment.

Medicaid insured or uninsured - The Dental Clinic at Primary Children's provides pediatric dental care for children ages 12 months to 14 years. Children with disabilities may be seen to age 18. This clinic also cares for children who have dental problems that must be corrected as necessary to support their other medical treatment.

Accepts private insurance, Medicaid and CHIP for any covered services, charging the appropriate co-payment amounts (which range from free to 50 percent of the amount, depending on the plan) instead of the flat fee.
Individuals with no insurance will be charged $100 for diagnostic tests and treatment of problems such as toothaches, broken teeth and infections. More complicated procedures will cost more.
