All facilities being built remodeled or re-opening under new ownership must meet current Salt Lake Valley Health Department standards set forth in Health Regulation 5.

Food Establishment Opening Check List

The following must be completed prior to opening:

  • Plan Review, where required.
  • Submit official letters from your municipality proving availability of sewer and culinary water connections (newly constructed facilities only).
  • Check with your local sewer district to determine grease trap requirements.
  • An application for a Business License in the municipality where business will be located.
  • Food Establishment Permit application must be completed and the fees paid.
  • Certified Food Safety Manager (“Serve Safe” or equivalent) registered with the Salt Lake Valley Health Department.
  • Pre-opening inspection by Salt Lake Valley Health Department inspector and all other required authorities (including but not limited to Fire Marshall and Building Inspector).

Note: Operating a food establishment without a valid Food Service Permit is a Class “B” misdemeanor.
