Food Establishment Plan Review
In Salt Lake County food service establishment owners, contractors, or design professionals are required to submit plans to the Bureau of Food Protection before constructing a new food service facility, converting an existing structure for use as a food service facility, remodeling of a food service facility, or changing the type of food service or food operation.
A plan review application (PDF 661 KB) and fee must be submitted to the Bureau of Food Protection before plans will be reviewed. The application consists of a Cover Sheet, a Risk Assessment Worksheet and an Operational Assessment (11 pages total). The application can be filled out on your computer and then printed and saved. The fee is calculated at the time of submittal in the Food Protection office using information supplied by the owner/operator on the application.

The Risk Assessment Worksheet and Operational Assessment must be completed by someone that has extensive knowledge of the establishment's food operations.
Plans will not be accepted unless accompanied by a completed application.
Only one set of plans is required to be submitted.
Please Note: Plan Review is only the first step in opening a new food establishment. After plans are approved and the establishment is built/remodeled, a Food Establishment Permit Application (PDF 165 KB) and fee must be submitted to the Bureau of Food Protection. Finally, a pre-operational inspection must be conducted to approve the permit.
