All organizations or persons conducting an activity or event involving large numbers of people are required to comply with Salt Lake Valley Health Regulations.
If your gathering lasts more than 2 hours and will be attended by more than 500 people, you must obtain a Temporary Mass Gathering Permit.
If your gathering has food service, you must obtain a Temporary Food Service Permit.
Gatherings held indoors or in outdoor facilities designed for large gatherings (such as stadiums) are generally not required to get a Mass Gathering Permit.

This form can be filled out, printed and saved. Completed forms can be faxed to the Bureau of Food Protection at (801) 313-6609 or emailed to dmoore@slco.org.

Fees will be determined upon receipt of application.

Fees

Fees for a Temporary Mass Gathering permit are based on Gathering attendance as follows:

Attendance	Fee
2-999	$250.00
1,000-4,999	$350.00
5,000-9,999	$450.00
10,000 or more	$500.00

A late fee of $175.00 applies if application is received less than 30 days prior to the gathering.
Application fees apply to all gatherings; no waivers will be granted.

The intent of the regulations is to protect the health and safety of the participants and those attending the gathering.
