NOTICE TO ALL FOOD SERVICE OPERATORS IN SALT LAKE COUNTY
Health Regulation 5 – 4.1.4: Managers Certified in Food Safety
Effective date is April 2006
Health Regulation 5 requires:
- Each food establishment that serves potentially hazardous foods shall employ at least one full-time manager certified in food safety at each establishment site.
- If the manager certified in food safety terminates employment, the establishment shall have sixty (60) days to employ a new manager certified in food safety.
To qualify as a manager certified in food safety, a person shall:
- Successfully complete a training course and pass an examination approved by the Utah State Department of Health.
- Make written application for certification to the Salt Lake Valley Health Department, 788 East Woodoak Lane, Murray, Utah 84107 along with an application fee of $15.00.
The certification shall be valid for three (3) years from the date of issue unless revoked or suspended by the Salt Lake Valley Health Department.
The following are currently approved by the Salt Lake Valley Health Department for training in Managers Certified in Food Safety:
