Health Regulation 5 requires:

• Each food establishment that serves potentially hazardous foods shall employ at least one full-time manager certified in food safety at each establishment site.
• If the manager certified in food safety terminates employment, the establishment shall have sixty (60) days to employ a new manager certified in food safety.

• Successfully complete a training course and pass an examination approved by the Utah State Department of Health.
• Make written application for certification to the Salt Lake Valley Health Department, 788 East Woodoak Lane, Murray, Utah 84107 along with an application fee of $15.00.

The certification shall be valid for three (3) years from the date of issue unless revoked or suspended by the Salt Lake Valley Health Department.
