Mail Requests

To request a vital record, please:

1. Complete the appropriate form: Birth, Death, Marriage Abstract, Divorce Abstract
2. Copy both sides of your acceptable photo ID(s)
3. Make a check or money order payable to SLVHD Vital Records
4. Send the above three items to:
   SLVHD Vital Records
   610 South 200 East
   Salt Lake City, UT 84111
5. Your mail request will be completed and returned promptly

Requests Must Include:

• Completed birth, death, marriage, or divorce request form
• A copy of both sides of your photo ID
• Your return address
• Your daytime telephone number is helpful

Please make check or money order payable to "SLVHD Vital Records." We do not accept credit card orders through the mail. Any request that is not complete or is missing a valid copy of the applicant’s ID or does not have the correct fee will be returned.
