Death Certificates
The Salt Lake Valley Health Department maintains death records going back to 1847 for deaths occurring within Salt Lake City limits.
We maintain death records for all of Salt Lake County beginning from September 1969. Our records also include computer death certificates for all Utah deaths from August 2006 to the present time.
Available death records include:
- 1847 – Present (Salt Lake City Limits)
- September 1969 – Present (All Salt Lake County Deaths)
- August 2006 – Present (All Utah Deaths)
Fees for certified copies of death certificates are listed on the death certificate request forms listed below.
Death certificates are protected records and are not available to the general public until 50 years after the death occurs. When the records have reached the open record status, copies are available to the general public at our offices or the County Records Center.
More recent records are available only to the immediate family as defined in Utah State Statute. Immediate family of the deceased is defined as: their parent, spouse, child, sibling, grandparent, grandchild. Others must prove a legal need to obtain a certified copy.
