Birth Certificate Services

One of the items that you will need throughout your life is a birth certificate.

Hospitals are required to file a birth certificate for each birth that occurs in their facility. To complete one, they give the mother a worksheet to collect the personal information and the new baby’s name. This worksheet, along with medical records of the mother and baby, are used to complete the birth certificate.

When the record is completed, it is submitted to the Utah Department of Health, Bureau of Vital Statistics. The record is then checked and registered into the state birth record system. Once the record is registered, it can be issued by any Vital Records office in Utah.

New Baby? If you have a new baby, birth records need to be requested by the parents and are not automatically issued. You should order a copy of your new baby’s birth certificate to ensure that complete and correct information was entered at the hospital.

Multiple Copies. Many people order more than one certificate when they place their order. Often the extra copy is kept in a safe deposit box or with a relative.

  • Driver license application
  • Passport application
  • Social Security
  • Retirement

Certified copies issued by Vital Records offices are legally valid.
